When available, the form accepts your name, email address or phone number, preferred response method, and a broad administrative category. It does not provide a message field and should not be used to share clinical details.

How the information is used

The submitted information is used to respond to a consultation, availability, fee, insurance, or other administrative question. It is sent to the practice as plain-text email.

How the site protects the form

The form uses a temporary session, a CSRF security token, rate limiting, a hidden spam field, and a timing check. These controls help reduce automated or unauthorized submissions.

Storage and email

The application does not write consultation submissions to its database, files, or a queue. The practice's configured email service provider processes the message for delivery. Email systems and normal hosting infrastructure may retain messages, technical logs, or operational records according to their own settings and policies.

Tracking and third parties

The practice pages do not use behavioral tracking, advertising pixels, social embeds, chat, maps, or CAPTCHA. Submitted information is not used for a sale and is not sold.

You may skip the form and call the practice directly. You may also use the approved contact details below to ask about access to or deletion of information held by the practice. Do not use this website for an emergency or to send clinical information.
